Living Well  is a chronic disease self management program (CDSMP) from Stanford University.   Workshops meet for two and  one-half hours, once a week for 7 weeks.  Participants will make weekly action plans, share experiences and help each other solve problems they encounter in creating and implementing their self management program.

Living Well...Take Charge of Your Health is open to anyone 18 years and older who are living with a chronic condition or caring for someone with a chronic condition.  Space is limited and registration is required.
